Central Communities Health Access Network (CC-HAN) exists to work with participating SoonerCare Choice Primary Care Providers to improve the access, quality, and continuity of care for SoonerCare Choice members.. Care management services are provided for members with complex health care needs. The care management team includes registered nurses as well as professionals from behavioral health, clinical social work, pharmacy, and other health disciplines.
